Inter-planar portals can occur naturally due to the movement of planes, mana fluxes and storms, and alignment of the stars.

Inter-planar portals do not occur naturally they are always caused by powerful mortals, immortals, or gods, many of whom work in mysterious ways.

Both, neither, I'm documenting thins encountered over the years, and throwing out ideas. These are not hard and fast rules. I figure planar portals work like dark matter does, ask any two theoretical physisists how dark matter works and you'll get three different answers, each with proofs to back them up.
